All requests for the use of BH Facilities must be submitted in writing (by completing the Application for use of School Facilities Form) and approved.
All organizations MUST follow COVID-19 Guidelines included in the updated Application for Use of School Facilities. This includes maintaing records of contact tracing and COVID questionnaires. Contact tracing forms and COVID questionnaire forms are included in the application link below.
Please submit completed form at least four weeks prior to requested use.
