Please be sure to complete the appropriate COVID-19 Screening Questionnaire BEFORE coming to school. 

• Teachers and staff will need to complete the questionnaire on a daily basis. 
• Parents and students will need to complete the questionnaire every Monday.

To access the appropriate questionnaire hover over the page title "COVID-19 Screening" , choose the appropriate questionnaire, complete by answering the questions and submit.
