Bus Change Information

 

Bus change information should be taken to the Main Office. Please be very specific when writing bus notes for changes. The following information should be included on the bus note:

 

1. Child's full name

 

2. The date

 

3. The person's name where the child is going

 

4. The address and telephone number for that person

 

5. Parent/Guardian Signature
